Position: Medical Social Worker – Home Health

Location: Pompano Beach, FL

Employment Type: Per Diem

Medical Social Worker Key Responsibilities:

• Assess patients’ psychosocial status in relation to illness, environment, and support systems
  
• Communicate findings to the Registered Nurse and interdisciplinary care team
  
• Develop and implement social service intervention plans
  
• Maintain accurate and timely clinical documentation
  
• Provide patients and families with guidance and referrals to community resources
  
• Serve as a liaison between patients, families, and community agencies
  
• Assist physicians and care team members in understanding social and emotional factors affecting health
  
• Participate in case conferences, care planning, and discharge planning
  
• Build and maintain relationships with community resources and agencies
  
• Perform additional duties as delegated by the Clinical Director/Supervisor
  

Qualifications:

• Master of Social Work from a program accredited by the Council on Social Work Education
  
• Licensed Clinical Social Worker (LCSW) required
  
• Minimum of one (1) year of healthcare experience
  
• Home health experience preferred
  
• Strong verbal and written communication skills
  
• Excellent organizational skills
  
• Current CPR certification
